Welcome to the on-line booking form.    Please be aware that certain tests are not available using our on-line booking service due to one or more of the following reasons:

• Due to the serious medical implications and devastating consequences that results from certain tests may give, eg tumour markers, HIV, such tests will only be entertained upon an appropriate referral from a medical practitioner; appropriate counselling may be required, both pre and post analytical.

• Certain tests require special handling and pre-analytical treatment and hence not suitable for sending through the postal system.

• Tests, such as 24 hour urine samples, are not suitable for sending through the postal system.

By virtue of having clicked through to this page, however, means that your test is available to be booked on line.   When your request is received by us, we will endeavour to send out the appropriate containers, packaging and a pre-paid return envelope.   If a blood sample is required, we will send out the necessary bottles and needles.   You will be required, however, to have the sample taken from a suitably experienced health care professional, eg GP , practice nurse, and they may charge a fee for this.

 

Having obtained the necessary samples, please ensure that they are posted back to the laboratory the same day to ensure samples are received fresh (try not to post on a Saturday, or over the bank holiday week-ends).

 

We do not process payments on-line so will send a payment form with the sample pack and payment will need to be sent back with the samples.
